  • * Maintain accurate and up-to-date patient records in accordance with HIPAA and other privacy regulations.
  • * Communicate with healthcare providers, patients, and family members to ensure seamless care transitions.
  • * Assist in the management of patient care plans, including education on health conditions, treatments, and medications.
  • * Monitor and assess patient progress, following up with patients as needed to ensure satisfaction and success in treatment.
  • * Handle incoming calls and inquiries from patients, addressing them in a courteous and professional manner.
  • * Work closely with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure holistic patient care.
  • * Provide emotional support and patient education, empowering patients to make informed decisions about their health.
  • Required Skills and Qualifications:
  • * Bachelors degree in healthcare, nursing, or a related field.
  • * Certification or training in patient care coordination is a plus.
  • * Strong communication and interpersonal skills, with the ability to interact effectively with diverse populations.
  • * Exceptional organizational and time management abilities.
  • * Proficient in using healthcare management software and Microsoft Office Suite.
  • * Strong attention to detail and accuracy in documentation.
  • * Ability to work independently in a remote environment while being a team player.
  • * Knowledge of medical terminology, healthcare insurance, and clinical processes is beneficial.
  • Experience:
  • * Minimum of 2 years of experience in patient care coordination, healthcare administration, or a related field.
  • * Prior experience working in a remote or virtual setting is preferred.
  • * Experience with electronic health records (EHR) systems is an advantage.
  • Working Hours:
  • * Full-time position.
  • * Flexible working hours to accommodate different time zones, with some evening or weekend hours required as needed.
