Job Summary

We're seeking an experienced Case Management Analyst to join our team on a part-time basis. As a Case Management Analyst, you'll be responsible for assessing the needs of individuals and developing personalized care plans to help them achieve their health and wellness goals. You'll work closely with a team of healthcare professionals, social workers, and other stakeholders to ensure that each individual receives the highest level of care and support.

Key Responsibilities

* Conduct comprehensive assessments of individuals' needs and develop personalized care plans to address their health and wellness goals

• Identify high-risk factors and service needs that may impact individual outcomes and collaborate with healthcare professionals to develop strategies for addressing these issues
• Facilitate and implement assigned care plan activities and monitor progress
• Collaborate with healthcare professionals, social workers, and other stakeholders to ensure that each individual receives the highest level of care and support
• Develop and maintain relationships with healthcare providers, community resources, and other stakeholders to ensure seamless care coordination
• Use case management and quality assurance processes to ensure compliance with regulatory and accreditation standards
• Participate in multidisciplinary case conferences to discuss individual cases and develop strategies for achieving optimal outcomes

Essential Qualifications

* Bachelor's degree in a related field (e.g., social work, psychology, public health)

• 2+ years of experience in case management or a related field
• Strong knowledge of case management principles and practices
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong analytical and problem-solving skills
• Proficiency in Microsoft Office applications (Word, Excel, PowerPoint)

Preferred Qualifications

* Master's degree in a related field (e.g., social work, psychology, public health)

• Experience in case management and release planning
• Supervisory experience in a healthcare or social services setting
• Certification in case management or a related field (e.g., CCM, ACM)
