We are seeking a compassionate and skilled PC Intensive Case Manager to provide trauma-informed case management services to clients in the Preferred Communities (PC) program. As a key member of our team, you will play a vital role in empowering individuals to reach their full potential and achieve self-sufficiency.

In this rewarding role, you will have the opportunity to work with a diverse population, including refugees, SIVs, asylees, parolees, and other eligible individuals. Your expertise and passion for delivering exceptional case management services will be essential in helping our clients navigate complex systems and access vital resources.

Key Responsibilities

  • Provide comprehensive case management services, including intake assessments, goal-oriented planning, and referrals to community resources
  • Develop and maintain positive relationships with clients, community partners, and internal stakeholders
  • Conduct cultural orientation workshops and provide guidance on accessing social services, employment, and education
  • Maintain accurate and timely records, including case notes and assessments, in our online database
  • Collaborate with internal teams and external partners to ensure seamless service delivery and coordination

Requirements

  • Bachelor's degree in Social Work, behavioral/human services, or a related field (or Associate's degree with 2 years of experience)
  • Case management experience, preferably with refugee or immigrant populations
  • Strong communication and interpersonal skills, with the ability to work with diverse clients and stakeholders
  • Proficiency in Microsoft Office applications and knowledge of community resources
  • Bilingual skills a plus
