Key Responsibilities:

• Deliver direct patient care, including medication administration and vital sign monitoring
• Develop and implement personalized care plans tailored to each resident's needs
• Collaborate with physicians, nurses, and therapists to ensure comprehensive care
• Maintain a safe and hygienic environment, adhering to infection control protocols
• Educate residents and families on health-related topics, promoting understanding and empowerment
• Maintain accurate and timely documentation in resident medical records

Required Skills:

• Current and unencumbered LPN license in the State of South Dakota
• Excellent communication and interpersonal skills
• Empathetic and patient-centered approach to care
• Strong team player with ability to work collaboratively

Qualifications:

• Completion of an accredited LPN program
• Experience in long-term care or similar healthcare setting preferred but not mandatory
