Job Description CVS Health is seeking a skilled and dedicated Remote Nurse Assessor to join our dynamic healthcare team. As a Remote Nurse Assessor, you will play a critical role in ensuring our members receive the highest quality care by conducting assessments and evaluations remotely. This position offers the flexibility of working from your home office, allowing you to maintain a healthy work-life balance while contributing to a leading healthcare organization. Requirements 1. Active Registered Nurse (RN) license in the United States. 2. Minimum of 3 years of clinical nursing experience. 3. Experience in assessment and care planning. 4. Proficiency in using electronic health records (EHR) systems. 5. Strong communication and interpersonal skills. 6. Ability to work independently and manage time effectively. 7. Reliable high-speed internet connection and a quiet home office. Responsibilities 1. Conduct comprehensive assessments of members’ health conditions and needs. 2. Develop individualized care plans based on assessment findings. 3. Coordinate care with other healthcare providers and community resources. 4. Monitor and evaluate members’ progress and make necessary adjustments to care plans. 5. Provide education and support to members and their families. 6. Document all assessments and care plans accurately in the electronic health record system. 7. Participate in team meetings and continuous education to stay updated with best practices. Associate Degree in Nursing (ADN) will be considered with relevant experience. Experience – At least 3 years of clinical experience is required. – Experience in conducting health assessments and care planning is highly desired. – Prior experience in a remote or telehealth role is a plus.
