This role involves managing patient care and coordinating services, with 8-hour shifts, 5 days a week, for a 13-week duration. Job Duties and Responsibilities of RN Case Management: • Coordinate patient care across multiple settings and services. • Develop and implement care plans tailored to individual patient needs. • Collaborate with healthcare professionals to ensure comprehensive patient care. • Monitor patient progress and adjust care plans as necessary. • Provide patient and family education regarding care plans and resources. Travel Requirements of RN Case Management: • Must live at least 60 miles from the facility to qualify for travel rates. Qualifications and Experience of RN Case Management: • 2 years of experience in case management or related field. • BLS certification. • National and state RN certifications as required.
