We are seeking a full-time, bilingual, and remote Licensed Vocational Nurse (LVN/LPN) to join our rapidly growing team as a Care Coordinator. You will be responsible for telephonically delivering high quality care, coaching, and appointment scheduling to assigned patients. Responsibilities:
- Remotely provide patient health coaching and care to improve patient outcomes
- Coach and educate patients on improving their Chronic Conditions, preventive care and physician directives
- Update and manage patient care plans based on assessment of patients’ needs and physician directives
- Uphold scheduling flexibility between the hours of 8:00am – 7:00pm (PST) to call patients who live in different time zones
Requirements:
- Valid Vocational Nurse (LVN/LPN) License
- Valid Vocational Nurse (LVN/LPN) License - IN THE SATE OF NEW YORK IS MANDATORY (NY LICENSE)
- 2 years+ Licensed Vocational Nurse (LVN/LPN) experience
- Bilingual (Fluent in English and Spanish)
- Experience working with EHRs/EMRs
- Strong knowledge and background of chronic conditions
- Ability to build rapport with patients
- Compassionate and caring demeanor
- Excellent documentation and verbal communication skills
- Intermediate to advanced computer skills with the ability to navigate multiple systems
- Reliable internet connection
