This is a unique opportunity to provide quality virtual care to patients ages 18 months and older, in a fully remote setting.At least four total licenses are required for this position. Must be licensed in at least one of these states: AZ, IL, CA, MO, MN. Must ALSO be licensed in at least three of these other states: AK, AZ, CA, HI, IL, MO, MN, MT, NM, OR, UT, WA.Position Summary:This Nurse Practitioner will deliver telehealth care to a diverse patient population through an integrated virtual care model. Responsibilities include clinical assessments, diagnoses, prescribing medications, and health education in a fast-paced, high-volume virtual environment.At least four total licenses are required for this position. Must be licensed in at least one of these states: AZ, IL, CA, MO, MN. Must ALSO be licensed in at least three of these other states: AK, AZ, CA, HI, IL, MO, MN, MT, NM, OR, UT, WA.Key Responsibilities:Provide virtual primary and episodic care to patients aged 18 months and olderPerform clinical assessments, diagnoses, and treatment using evidence-based guidelinesPrescribe medications appropriatelyDocument clinical encounters in EHREducate patients on chronic condition management and general wellnessCollaborate with clinical support teams when necessaryAt least four total licenses are required for this position.
