Remote RN Care Manager - WFH

Position Overview Steward Health Care is seeking a compassionate and experienced Remote RN Care Manager to join our team. In this role, you will utilize your clinical expertise to assess, coordinate, and manage patient care remotely, ensuring optimal health outcomes. The ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for patient-centered care in a virtual setting.
Key Responsibilities
Develop, implement, and evaluate individualized patient care plans. Provide education, resources, and emotional support to patients and their families. Collaborate with interdisciplinary teams to coordinate comprehensive care. Monitor patient progress and modify care plans as necessary to meet their needs. Deliver care management services using telephonic and electronic systems. Ensure compliance with all organizational policies and regulatory standards.
Qualifications
Licensure: Current and unrestricted RN license. Experience: Minimum of 3 years of clinical experience, preferably in care management, case management, or home health. Knowledge: Strong understanding of chronic disease management and care coordination principles. Skills: Proficiency with electronic medical record (EMR) systems; exceptional organizational and communication skills. Independence: Ability to work autonomously while managing multiple priorities effectively.
