About the position Nightingale Homecare is seeking dedicated Registered Nurse (RN) Case Managers for both full-time and part-time positions within our home health agency. As an RN Case Manager, you will have the flexibility to set your own schedule, primarily conducting patient home visits during weekdays between 8:00 am and 5:00 pm. This position requires a minimum of one year of acute-care nursing experience and a valid RN license in good standing. We offer a competitive salary, with full-time positions earning up to $90,000 annually, along with a generous benefits package that includes a $4,000 sign-on bonus, health insurance, dental insurance, vision insurance, and a 401(k) plan. Responsibilities • Develop and maintain one-on-one relationships with patients. , • Teach patients how to manage their health issues effectively. , • Celebrate patient successes in healing and recovery. , • Conduct patient home visits primarily during weekdays. , • Complete OASIS documentation with significant support. , • Work independently while managing patient care. Requirements • Minimum 1 year of acute-care nursing experience. , • Registered nurse license in good standing. , • Current CPR certification. , • Current TB test and Fingerprint Card. , • Current AZ driver's license and auto insurance. , • Reliable vehicle for driving to client locations. , • Ability to work independently.
