Field Clinical Coordinator in Central TN

Positions in this function address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities. • Generally work is self-directed and not prescribed • Works with less structured, more complex issues • Serves as a resource to others • Assess, plan and implement care strategies for transition that are individualized by a patient and directed toward the most appropriate, lease restrictive level of care. If you are located in central TN, you will have the flexibility to work remotely* as you take on some tough challenges. Primary Responsibilities: • Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services • Manage the care plan throughout the transition between groups process as a single point of contact • Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members • Advocate for patients and families as needed to ensure the patient’s needs and choices are fully represented and supported by the health care team • Advocate for transitions of care for those individuals institutionalized long term to return to the community for their care • Coordinate multiple resources and providers to develop a solid person centered support plan for care in the community • Ability to communicate effectively with Behavioral Health Teams, Provider Relations team, Member Advocacy team, Long Term Service s and Supports Field Staff and their managers to provide the highest quality support plans for CHOICES Individuals You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: • RN or Social Worker license in the state of TN or clinical degree • 5+ years of experience working with MS Word, Excel and Outlook • 3+ years of experience working within the community health setting in a health care role • Experience in long-term care, home health, hospice, public health or assisted living • Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices • All employees working remotely will be required to adhere to UnitedHealth Group’s Telecommuter Policy At UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
