LVN Case Manager: Maternal Health Nurse

About the position The Licensed Vocational Nurse (LVN) Case Manager at Molina Healthcare is responsible for assessing, planning, and coordinating care for members with high needs, particularly in maternal health. This remote position requires excellent communication and computer skills to manage a caseload effectively, ensuring members receive quality care that is both medically appropriate and cost-effective. The role involves collaboration with healthcare professionals and the use of motivational interviewing techniques to support members in achieving their health goals. Responsibilities • Complete clinical assessments of members to determine eligibility for case management. , • Develop and implement case management plans in collaboration with members and healthcare professionals. , • Conduct telephonic, face-to-face, or home visits as required. , • Monitor care plans to evaluate effectiveness and document interventions. , • Maintain an ongoing member caseload for regular outreach and management. , • Promote integration of services including behavioral health care and long-term services. , • Facilitate interdisciplinary care team meetings and collaboration. , • Use motivational interviewing to educate and support members during contacts. , • Assess barriers to care and provide coordination and assistance to members. Requirements • Completion of an accredited Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) Program. , • 1-3 years of experience in case management, disease management, managed care, or medical/behavioral health settings. , • Active, unrestricted LVN/LPN license in good standing. , • Valid driver's license with a good driving record. Nice-to-haves • Bilingual in Spanish (preferred but not required). , • 3-5 years of experience in case management, disease management, managed care, or medical/behavioral health settings (preferred). , • Compact Nursing licensure (preferred).
