Nurse Case Manager

BlueCross BlueShield of South Carolina is looking for a Nurse Case Manager to join our team and help us provide comprehensive and quality care for our members. The Nurse Case Manager will collaborate with providers, members, and families to develop, implement, and monitor plans of care that are tailored to the specific needs of each individual. The ideal candidate will have a passion for helping others and a commitment to ensuring that the best quality of care is received. The successful candidate will be responsible for coordinating all aspects of care and monitoring the progress of members to ensure their recovery. This position requires a current RN license in South Carolina and a minimum of 3 years of experience in case management. Candidates must also demonstrate strong problem-solving skills, excellent communication and organizational skills, and the ability to work in a fast-paced environment. Responsibilities: Develop, implement, and monitor customized plans of care for members, providers, and families. Coordinate all aspects of care including monitoring progress of members and ensuring they receive the best quality of care. Utilize problem-solving skills to address any issues that arise. Maintain strong communication and organizational skills. Remain abreast of current trends and best practices in case management. Work in a fast-paced environment and meet deadlines. Adhere to all applicable laws and regulations, as well as BlueCross BlueShield of South Carolina policies and procedures. Maintain current RN license in South Carolina.
