Position Summary: TheClinical Consultant Pharmacistis responsible to draw from geriatric-focused clinical practice guidelines and collaborate inside multidisciplinary teams to deliver clinical recommendations and process guidance for a portfolio of long-term care, assisted living, and independent living homes to optimize for resident outcomes, safety, and regulatory compliance. The compensation is salary. The environment is hybrid work-from-home and on-site at the care homes. TheClinical Consultant Pharmacistworks in partnership with the clinical team, operations, home partners, and other departments in the CareRx organization. This position can be FULL-TIME 5 days per week, or PART-TIME 1-3 days per week and is located in Calgary with required travel to Lethbridge area. The Clinical Consultant Pharmacist is responsible for completing clinical pharmacy activities in alignment with the Continuing Care Health Service Standards, Site Policy, Internal Policies and the Ministerial Order of Clinical Pharmacy Services Your Day to Day: • Participate and/or lead Quality and Medication Safety committee meetings when applicable • Complete medication cart and medication room audits quarterly. • Participate in influenza and COVID preparedness activities -- obtaining dose-appropriate Tamiflu orders and coordinating and carrying out on-site clinics in partnership with nurses at supportive and assisted living care sites and if applicable long-term care homes. • Follow and adhere to internal policies and processes established by the Clinical Department and the broader organization. • Maintain resident and employee confidentiality in accordance with organizational policy and professional/legal obligations • Support best practice medication management processes. • Establish and maintain effective working relationships with physicians, nurses, and other care practitioners such as dieticians, and social workers with a focus on optimizing medication therapy and wellness. • Serve as a representative and leader of the medication management aspect of CareRx services at the homes. • Aid in operational activities through coordinated service delivery, supporting site to pharmacy communications, and ensuring continuity of care for residents. • Provide on-call services within the team rotation where applicable What You Will Bring to The Team: • Long Term Care/Assisted Living Pharmacy experience would be an asset • Authorization to Administer Drugs by Injection • Additional Prescribing Authority (APA) would be an asset • Competency with computers IE: Excel, Kroll • Excellent time-management and organizational skills • Self-driven and self-managed • Accurate, precise, dependable • Ability to drive within the Lethbridge and/or Southern Alberta Region Qualities for Success: • Excellent interpersonal skills • Ability to manage changing priorities • Ability to work independently, and as a member of a team • Customer service oriented • Excellent problem-solving and decision-making skills • Strong attention to detail • Strong organizational and time-management skills • Curiosity to develop your career in the geriatric-focused, specialty pharmacy sector of pharmacy What you will love about working for CareRx: • Training in specialty pharmacy • Professional Licensing, Membership, and Insurance reimbursement • RRSP Matching • Perkopolis Employee Discount Program • Corporate Gym Membership • Continuing Education and Professional Development • Employee Recognition and Service Awards • National Culture Events (Holidays, Team Lunches, Contests, Wellness) • Employee and Family Assistance Program (EFAP) • Internal growth opportunities Application Process: CareRx welcomes and encourages applications from people with disabilities.
