About the position The LTSS Service Coordinator is responsible for managing a defined caseload of individuals in specialized programs, focusing on service coordination functions. This role involves conducting assessments, developing comprehensive support plans, and collaborating with interdisciplinary teams to ensure integrated care for members with long-term services and supports (LTSS) needs. The position requires both home-based work and field visits to member homes or nursing facilities. Responsibilities Conduct face-to-face program assessments using various tools and motivational interviewing techniques. , Identify members with potential clinical health care needs and coordinate their cases with the clinical healthcare management team. , Manage non-clinical needs of members with chronic illnesses and disabilities to ensure efficient utilization of services. , Document short and long-term service and support goals in collaboration with the member's care team. , Identify members that would benefit from alternative services or waiver programs. , Serve as a mentor or subject matter expert for new staff and assist in training associates. , Submit utilization/authorization requests to management with supporting documentation. , Report critical incidents to appropriate internal and external parties. , Assist in appeals, grievances, and state audits. Requirements Experience in service coordination or case management for individuals with disabilities or chronic illnesses. , Knowledge of long-term services and supports (LTSS) and behavioral health (BH) needs. , Strong communication and interpersonal skills to engage with members and their support teams. , Ability to conduct assessments and develop comprehensive support plans. Nice-to-haves Experience with motivational interviewing techniques. , Familiarity with state and federal regulations regarding LTSS and BH services.
