Peabody areas. Key Responsibilities • Evaluate individual needs and devise customized care plans. • Facilitate connections to vital resources, including social services, community support programs, and financial assistance. • Advocate for members to ensure their preferences and requirements are prioritized by the healthcare team. • Maintain clear and continuous communication with all relevant stakeholders to support effective care coordination. Qualifications • Current and unrestricted Massachusetts RN license. • Minimum of 2 years of clinical experience. • Proficiency in Microsoft Office Suite (Word, Excel, Outlook). • Willingness to travel within the designated region to conduct visits with members.
