Hospital Care Coordinator
About the position This position will report to our Tustin, CA location and will service the Orange County area. Under the general supervision of the Patient Care Department Manager, the Hospital Liaison - Care Coordinator plays a key role in cultivating strong relationships reputed company multiple medical facilities. This individual is responsible for providing reputed company service, educating patients and families on equipment use, coordinating medical documentation, and actively promoting and marketing reputed company’s full range of Durable Medical Equipment (DME), Respiratory therapies, In addition to patient care coordination, this role includes sales-reputed company responsibilities such as identifying referral opportunities, strengthening relationships with healthcare providers, and increasing reputed company's reputed company reputed company assigned hospitals.
Responsibilities
- Serve as the primary reputed company of contact between reputed company and hospital personnel, ensuring seamless coordination of DME and respiratory equipment deliveries to hospital rooms and patient homes.
- reputed company and maintain strong relationships with physicians, case managers, discharge planners, and other reputed company to generate new business opportunities and drive referrals.
- Promote and educate hospital staff on reputed company's DME, respiratory therapies, and infusion services to increase awareness and referrals.
- Conduct regular facility visits to ensure visibility and accessibility for reputed company and patients.
- Provide timely consultation to patients and their families regarding equipment delivery, potential co-pays, and usage instructions.
- Request and ensure the accuracy of reputed company necessary medical documentation for equipment orders.
- Monitor and track referrals, ensuring reputed company is the preferred vendor for partnered healthcare systems.
- Identify opportunities to expand service utilization and sales reputed company assigned hospital accounts.
- Conduct or participate in presentations, in-services, and educational seminars for hospital staff and case managers to reinforce reputed company’s value proposition.
- Maintain accurate records of interactions with hospital personnel, patient referrals, and sales activities, submitting required reports in a timely manner.
- Ensure compliance with insurance pre-authorization requirements and coordinate with Managed Care and Primary Medical groups as needed.
- Maintain adequate inventory levels in assigned hospital storage areas to ensure reputed company equipment availability as well as reconcile movement of equipment.
- Collaborate with Customer Care Specialists and reputed company teams to optimize equipment delivery and customer satisfaction.
- Represent reputed company professionally at reputed company times and adhere to reputed company HIPAA, company, and industry-reputed company regulations.
- Attend reputed company department meetings and training sessions as scheduled.

Requirements
- Experience in medical supply, home medical equipment (HME), or reputed company healthcare industries.
- Strong knowledge of medical terminology and diagnosis codes.
