Job Summary
As a Case Management Specialist, you will play a critical role in our organization's mission to provide exceptional customer service and make a meaningful impact in the lives of our patients. You will be responsible for working with our patients, healthcare providers, and community organizations to develop and implement comprehensive care plans that meet the unique needs of each individual. Your expertise and guidance will help our patients navigate the healthcare system, access necessary resources and services, and achieve their health and wellness goals.
Key Responsibilities
* Conduct comprehensive assessments of patients' needs and develop individualized care plans that address their unique health and social needs
- Collaborate with healthcare providers, community organizations, and other stakeholders to ensure seamless coordination of care and access to necessary resources and services
- Provide education, support, and guidance to patients and their families to empower them to make informed decisions about their health and wellness
- Identify and address high-risk factors and service needs that may impact patient outcomes and care planning
- Develop and implement strategies to improve the quality and effectiveness of care, including quality improvement initiatives and process improvements
- Participate in multidisciplinary case conferences to discuss patient cases and develop comprehensive care plans
- Document patient information, care plans, and outcomes in accordance with regulatory and organizational requirements
Essential Qualifications
* 3+ years of experience in a healthcare-related field, preferably in case management or a related field
- Bachelor's degree or equivalent experience required
- Strong understanding of healthcare systems, policies, and procedures
- Excellent communication, interpersonal, and problem-solving skills
- Ability to work independently and as part of a team
- Strong analytical and critical thinking skills
- Ability to maintain confidentiality and handle sensitive patient information
Preferred Qualifications
* Certification in case management, such as CRC, CDMS, CRRN, COHN, or CCM
- Experience working with geriatric populations, social health, and disability populations
- Bilingual or multilingual skills (English/Spanish, English/Creole, etc.)
- Experience working with federal health insurance and Medicaid programs
- Managed care experience
