Provide primary healthcare services to patients in their homes, including routine check-ups, diagnoses, and treatments. Collaborate with interdisciplinary team members, such as nurses and therapists, to coordinate and implement patient care. Monitor and evaluate the effectiveness of treatment plans and make any necessary adjustments. Educate patients and their families on disease management, preventive care, and healthy lifestyle choices. Maintain accurate and up-to-date medical records and documentation. Adhere to all HIPAA and privacy regulations to protect patient confidentiality. Stay informed of current medical practices and advancements in the field to provide the best possible care to patients. Act as an advocate for patients, ensuring their needs and concerns are addressed. Participate in quality improvement initiatives and provide feedback to enhance the home care program. Communicate effectively with patients, families, and other healthcare professionals to ensure continuity of care. Adhere to all company policies, procedures, and ethical standards. Attend meetings and trainings as required to stay informed and up-to-date on company policies and practices. Maintain a positive and professional attitude while representing the company and its values.
