INTERNAL ONLY: PHN Supervisor for Home Visiting Program
About the position Lorain County Public Health is seeking a dynamic, community-focused Registered Nurse (RN) to join our team as a Nurse Supervisor within the Community Health Division. This leadership role supports our nurse home visiting programs—Family Connects Ohio (FCO) and Plan of Safe Care (POSC)—which play a vital role in improving outcomes for families across our community. The Nurse Supervisor will lead day-to-day operations while driving the planning, coordination, and implementation of innovative, high-impact services and initiatives. Working within the scope of nursing practice and under the direction of the Community Health Director, this position offers the opportunity to make a meaningful difference in the lives of children and families while guiding a dedicated team of professionals.
Responsibilities
- Coordinate and ensure compliance with staff orientation, ongoing training and credentialing of program staff
- Maintain staff training records and requirements
- Supervise and evaluate Public Health Nurses and assigned staff
- Oversee scheduling processes and assignment of home visits
- Implement quality assurance and performance improvement activities.
- Conduct regular chart reviews and field observations to ensure compliance with agency standards and FCO program fidelity.
- Coordinate weekly case conference meetings.
- Maintain regular communication with Family Connects International Implementation team
- Monitor program performance metrics and prepare required reports.
- Develop and monitor program budgets
- Coordinate and oversee the Fetal Infant Mortality Review (FIMR) Board
- Plan, coordinate and execute initiatives of the Cribs for Kids grant
- Prepare and submit grant applications, work plans, budgets and reports
- Seek out and apply for additional sources of funding to support programs
- Develop and maintain partnerships with local healthcare providers and community stakeholders to inform about public health programs and collaborate on mutual objectives.
- Participate in public health emergency response efforts as required.
- Other public health duties as assigned by the Director and/or Health Commissioner
Requirements
- Must be licensed registered nurse pursuant to Section 4723.13 of the Revised Code.
- Must be able to travel with reliable transportation.
- A valid Ohio driver's license with ongoing proof of auto insurance.
- Current CPR certification.
- Proof of up-to-date vaccinations for healthcare providers or in process of completion.
Nice-to-haves
- Bachelor’s degree in Nursing with five (5) years’ experience, public health, preferred.
- Leadership or management experience (or equivalent education and training).
- Experience writing and/or working with grants, contracts and policies, and a basic understanding of budgets.
- Experience working collaboratively on teams.
- Must have clear oral and written communication skills and be competent with computers, including electronic documentation, and Microsoft Office Suite applications.
