- You will be completing psychosocial assessments on patients. The assessment shall include, but is not limited to assessment of the patients lifestyle, coping, support systems and needs. Demonstrate a basic understanding of psychosocial issues related to hospitalization for patients of any and all age populations
- You will be creating and facilitating timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. You will also facilitate patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput
- You will be providing emotional support, psychosocial assessment, supportive counseling, crisis intervention, group therapy and the provision of information and referrals to appropriate community resources
- You will be participating in formulating policies and procedures to facilitate maximum integration of clinical social work services with other disciplines. Demonstrate knowledge of social service policies and procedures. Provide staff with in-service education as needed
- You will be developing relationships with community agencies and serve as liaison to coordinate continuity of care for patients. Plan and coordinate with staff of community facilities, social service agencies, health department and other community agencies and resources to meet patients post hospitalization needs as appropriate
- Graduate of a school accredited by the Council on Social Work Education
- MSW (Masters Degree in Social Work or equivalent) or BSW with commensurate experience required
- Current State of Nevada Board of Examiners for Social Work license
- Minimum of two (2) years full-time social work experience preferred
- Medical social work experience preferred
- Certification in social work or case management preferred
- Knowledgeable about internal, community and post-acute care resources
- BLS certification required within thirty (30) days of hire
