REMOTE: Part-Time Medical Assistant for Remote Patient Monitoring (RPM) Program Needed. Work from Home, Weekends Required.

Purpose/belief statement: Convergence Telehealth is currently recruiting part-time remote Care Management Team- Medical Assistant (MA) with a passion for delivering a high-quality customer experience, to support our clients. As a Care Management Team- Medical Assistant, you will be involved in remote patient monitoring (RPM). Familiarity and experience with remote patient monitoring is very highly desired and also the knowledge of setting up patients on at home blood pressure devices and glucose monitors, as well as various other devices. Comfort with technology is required, with the ability to help patients set up their at home devices remotely for both android and Apple devices. The position is part time, with the ability to grow to full time. The position is 5 days a week, and weekend work is required for 4-5 hours on both Saturdays and Sundays, as well as some weekday hours as well. There is no work on holidays. This is a remote position that can be based anywhere in the United States. Equipment such as a laptop, headset and monitors will be provided. Job duties & responsibilities

• Identifies patient/family education needs and ensures that patient/family members have adequate information to participate in remote monitoring.
• Monitors daily vitals sings of patients, engages with patients in assessing their physical state and escalates as needed.
• Works collaboratively with the team to engage monthly with patients.
• Interprets screening and selective laboratory/diagnostic tests.
• Initiates and maintains communication and collaboration with physicians, social workers, care team leaders, staff nurses, other care giving disciplines, and patients/families to develop, implement, and evaluate a transition plan of care for each patient.
• Assesses financial and insurance resources to maximize the health care benefit to the patient.
• Monitors the achievement of clinical outcomes and communicates with care teams, primary and specialty physicians and staff, regional providers, and community resources.
• Assesses complexity of care needs and potential/actual issues or gaps in care.

Qualifications

• Certified Medical Assistant from an accredited MA program
• Excellent communication skills, problem solving and conflict resolution skills
• Computer skills in word processing, database management, and spreadsheets
• 1+ years of relevant RPM experience

PREFERRED

• Previous experience in one or more of the following: Case Management, Transitional Care Management, Remote Patient Monitoring and Chronic Care Management
• Bilingual in Spanish is highly desired but not required

Knowledge/Skills/Abilities Work environment

• May work beyond normal working hours, on weekends and holidays, when necessary.
• Is subject to frequent interruptions.
• Field based work required. Occasional overnight travel will be required as times with field based assignments.

• Ability to lift, push, pull, and move a minimum of 50 pounds.
