Job Summary

As a Remote RN Case Manager, you will be responsible for:

• Patient Assessments: Perform comprehensive patient assessments and develop individualized plans of care
• Care Coordination: Collaborate with the care team to coordinate patient care and services
• Patient Education: Teach patients and families about self-care techniques and promote health and independence
• Supervision: Supervise LPNs and HHAs to ensure high-quality patient care
• Documentation: Complete accurate and timely documentation
• Communication: Regularly communicate patient progress to the clinical manager and care team
• Staff Education: Plan and provide staff education to ensure ongoing learning and development
• On-Call Responsibilities: Participate in on-call responsibilities and provide services to patients and families as needed
• Clinical Development: Participate in clinical development and continuing education programs to stay current in your field

Requirements

To be successful as a Remote RN Case Manager with Amedisys, you will need:

• RN License: Current RN license in the state(s) you are assigned to work
• CPR Certification: Current CPR certification
Clinical Experience:
• One (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical.*
• If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program.
• Driver's License and Transportation: Valid driver's license, reliable transportation, and liability insurance
