Key Responsibilities

 As a Medicare Population Health Nurse Case Management Analyst at Cigna, you will:

 Patient-Centered Care: Provide patient-centric, evidence-based care management to Medicare beneficiaries, ensuring they receive the highest quality of healthcare services.

 Assessment and Analysis: Conduct comprehensive assessments of patient health and formulate care plans to address their specific needs, collaborating with healthcare providers and patients.

 Data-Driven Decision-Making: Utilize data analytics to identify trends and opportunities for improving patient outcomes and the cost-effectiveness of care delivery.

 Interdisciplinary Collaboration: Work closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, social workers, and pharmacists, to coordinate and enhance patient care.

 Educational Support: Educate patients on their conditions, medications, and self-management strategies to promote healthier lifestyles and optimize health outcomes.

Required Skills and Qualifications

 To excel in this role, you should possess the following skills and qualifications:

 Registered Nurse (RN): Hold a valid RN license.

 Case Management Expertise: Proven experience in case management, preferably in a healthcare setting.

 Medicare Knowledge: In-depth knowledge of Medicare regulations, guidelines, and reimbursement processes.

 Data Analytics: Proficiency in data analysis to drive informed decisions and continuous improvement.

 Communication Skills: Exceptional interpersonal and communication skills to effectively engage with patients and the healthcare team.

 Compassion: A deep commitment to delivering patient-centered care with empathy and compassion.

 Adaptability: Ability to thrive in a fast-paced, evolving healthcare environment.
